conducted on February 25, 2026 in relation to my workplace acoustic injury at BCRTC OMC1, 6800 14th Ave,
Burnaby.

During our February 19, 2026 call you advised me to report fire and building code violations to the City of
Burnaby independently. | did exactly that. The Assistant Chief Building Inspector — a Professional Engineer — is
now actively investigating the same installation your report treated as a legal, compliant workplace.

This correspondence and the attached documents have been submitted to my WorkSafeBC case manager, her
supervisor, and are part of my active claim file #42647461.
