exposure

The missing server panel: Photographed January 29, 2026 — an engineered acoustic
control that was absent during the period of my injury

The January 29 Acoustic Trauma Brief: Filed with WorkSafeBC before the worst
symptoms began

A workplace injury investigation that omits the clinical evidence of the injury it was
commissioned to investigate is not a valid basis for claim adjudication.

FORMAL REQUEST

| am requesting that WorkSafeBC:

1: Note this formal statement in my claim file as evidence that Austin Puder had direct
personal knowledge of my workplace ear injury on January 27, 2026 — prior to the
commissioning of the Arcose noise survey

2: Note that Arcose Consulting was not provided with my January 29 Acoustic Trauma
Brief, my audiogram, or my clinical diagnosis when conducting their survey

3: Note that the WorkSafeBC Prevention Services inspection report contains material
omissions regarding the clinical evidence of my injury

4: Note that the union representative Rabjit Bul was present at the January 27 meeting
and can corroborate this account
5: Ensure the medical advisor reviewing this claim has access to this statement, my
January 29 Acoustic Trauma Brief, my audiogram, and my full clinical record — not only
the employer's investigation report

6: Activate interim wage loss payments — | have received no income since early
February 2026 while adjudication continues
