|. Introduction & Professional Profile

| am submitting this statement to document a serious, preventable workplace injury. | ama
25-year heavy industry veteran with professional experience (during childhood) in recording
studio environments. My background provides me with an expert-level understanding of
acoustic resonance, signal-to-noise ratios, and frequency-specific hazards. Until the current
assignment at Workstation 3, in the OMC1 Training room, my hearing health has been stable
and impeccable, as evidenced by three years of clean audiometric baseline records held by
WorkSafeBC.

Il. Timeline of Employer Negligence & Prior Notice
This injury is the direct result of an unaddressed environmental hazard

e@ Formal Notice: On Jan 22 2026, | filed a Safety Concern Form identifying the
unshielded server rack at Workstation 3 as a hazard. At that time, | was already
experiencing warming symptoms, including persistent headaches.

e Failure to Remediate: The employer failed to replace the server side-panels or
mitigate the resonance. Consequently, my condition progressed from systemic
headaches to acute physical trauma, including Tonic Tensor Tympani Syndrome
(TTTS) and stabbing pain in the left ear.

Ill. Scientific Methodology & Evidence of Injury

Using an external USB connected microphone (Neewer Large-Diaphragm Condenser
Microphone), | have quantified the hazard that the employer failed to manage. My system
was calibrated against 440 Hz and 1000 Hz reference tones (recorded in a small closet full
of clothing and stuffies) to ensure data integrity.

e@ The 328 Hz Spike: My data identifies a massive mechanical resonance at 328 Hz.

e The Reflection Effect: A glass window at my left acts as an “acoustic mirror,"
reflecting this energy into my left ear.

e The Absorption Proof: My comparative data shows a 70dB drop in energy "Behind
Head." This proves my skull is physically absorbing the "missing" energy—the direct
physiological cause of my chronic headaches and neurological exhaustion.
