EXHIBIT F

Describe your safety concern or near miss in detail (describe what is wrong, including risk to
workers/customers. example: leaking pipe causing water to pool on floor.) *

| would like to bring to your attention the issue of ambient noise levels in the Control Training Room on the
floor. 2.0f OMC 1. There is 2, large, floor mounted servers in the back corner of the room, behind work
station 3. While the noise levels are not high enough by Work Safe BC standards to require hearing
protection (85dB during 8hrs), they are much too high for an office or learning environment. My phone dB
meter, while sitting at my assigned Work Station (3), is read upward of between 60 - 70db ambient noise
level. Even with active noise canceling headphones, the noise is still quite loud. Once ambient noise level
exceeds 55 - 60 dB, it can interfere with concentration, and can increase stress during mentally
demanding tasks. It makes it.especially hard for the student at Work Station 3 to hear the instructor. | feel
this noise level has many detrimental effect to all students in the room, but especial those at work station
3. Work Station 3 is at-particular disadvantage, due to the much higher ambient noise. | am prone to
migraines and take steps at home and at work to minimized my exposure to excessive noise. | can not
wear noise canceling headphones while trying to listen to the instructor in the training room. | feel this is
not a good place to have a server rack, with the excessive noise and heat that it produces. It also

significantly reduces the space behind work station 3, as compared to other work station.

Was this an isolated event or does this happen
regularly? *

Immediate Actions Taken (what did you do to try and fix/mitigate the hazard? example: | mopped up and
secured the area of the incident) *

| would like to see sound level testing done in Control Training room, especially from work station 3.

What do you suggest as a permanent / long term solution? (example: contacted Maintenance to replace
leaking pipe)

Take significant steps to reduce the noise form the server (with out further reducing space behind work
station 3), or have the server moved somewhere else entirely.
