Location: 6800 14th Ave (OMC1) - Unpermitted Industrial Installation Days Since
Injury (Jan 23): 55 Days Since Specialist Requested (Jan 30): 48 Days Past 6-
Week Acute Treatment Window (March 6): 13 OFFICIAL STATUS: "Both ENT
and Neurology does not think outpatient follow up is warranted at this time." (ER
Physician Note) CLINICAL DISCREPANCY: Records claim hearing tests were
"completely normal." Verified data shows a 10dB drop in left ear (8kHz range)
compared to September 2025 baseline. REGULATORY STATUS: "Your concerns
have been forwarded to the Building Inspections Supervisor for review and further
