THE PROGRESSION β€” INTERNAL CHANNELS EXHAUSTED

| want to be unequivocal about the sequence of events. | did not report to WorkSafeBC as a first
resort. | followed the correct progression through every available internal channel before | was
forced to protect myself legally.

* Step 1: Raised concerns verbally with my instructor from the first week of training.

* Step 2: Filed formal MyHSE Safety Concern #75625 on January 22 β€” the employer's
own safety reporting system.

* Step 3: Communicated directly with my instructor in writing on January 23, 25, and 26 β€”
reporting physical symptoms, requesting remediation, raising methodology questions.

* Step 4: Attended the January 27 meeting with Austin Puder, Breanne Douglas, and
union representative Rabjit Bul β€” formally reported ear injury, requested internal
escalation to workplace injury.

* Step 5: Was told by Austin Puder to take it to WorkSafe.

* Step 6: Filed WorkSafeBC Unsafe Workplace Report on January 29 β€” only after being
directed to do so by management, and only after my physical condition had deteriorated
to the point of an Emergency Room visit.

At every step, the employer minimized, redirected, or closed the concern. The safety concern
was closed without remediation. The meeting produced no accommodation. No EFAP referral
was offered at any point. When my health finally collapsed to the point where | could no longer
attend class, | filed with WorkSafeBC as directed. The WorkSafeBC Prevention Services
investigation that followed contained material omissions. The investigator had access to my
January 29 Acoustic Trauma Brief. His report does not reference it. He advised me on February
19 to report fire and building code violations to the City of Burnaby independently. | did exactly
that.
