ESOPHAGOGASTRODUODENOSCOPY PROCEDURE REPORT PROCEDURE DATE: 08/03/2022

ENDOSCOPIST: Kenneth Atkinson STATUS: inpatient

INDICATIONS: The patient is a 43 yr old male here for an EGD due to therapeutic Procedure.
PROCEDURE PERFORMED: EGD with control of bleeding

MEDICATIONS: Fentanyl 100 meg IV and Midazolam (Versed) 5 mg IV
TOPICAL ANESTHETIC: Benzocaine Spray

CONSENT: Informed consent was obtained from the patient or representative prior to the procedure. The risks of the procedure and
possible altematives have been discussed. - Rare tisks [1:3000] of serious adverse events were cited including reaction to sedation,
bleeding and perforation. - Due to the anticipated need for therapeutic intervention such as esophageal dilation, removal of foreign

body, variceal banding or other intervention an appropriate discussion was held surrounding the increased risk of adverse events,

DESCRIPTION OF PROCEDURE: During pre-op preparation period all mechanical & medical equipment was checked for proper
function. After the risks, benefits and alternatives of the procedure were thoroughly explained, informed consent was verified and
confirmed. The Olympus GIF 1TH190 SN 2942890 endoscope was introduced through the mouth and advanced to the second portion of
the duodenum. Retroflexion was performed in the stomach and revealed no abnormalities. The gastroscope was then slowly withdrawn

and removed.
ESOPHAGUS: The esophagus appeared normal.

STOMACH: The fundus, cardia, body and antrum of the stomach all appeared normal. The stomach distended normally and the

endoscope passed easily though the pylorus into the duodenum.

BUODENUM: A small bleeding, irregular shaped and deep ulcer with a visible vessel and active bleeding was found in the 1st part of the
