He reports the pain is intermittent when it does occur and is often aggravated with deep inhalation. He has not noted any other aggravating factors. He began taking a muscle relaxant plus aspirin combination approximately 5 weeks ago for management of this pain. He reports that on occasion, the muscle relaxant he uses also comes with an ibuprofen combination. He has been taking a 500 mg tablet 3 to 4 times a day for relief of this pain. He has been taking this medication for the last 5 weeks. He has also noted some relief frorn Gaviscon since Saturday, March 5, 2022 (he was prescribed this in the emergency department when he presented with this centrai substernal chest pain). He does not endorse any other associated features accompanying the pain. Occasionally, when the pain is 9/10, he does report some nausea. Mr. Holand had presented to the emergency department with this chest pain on March 5, 2022, as mentioned above. At this time, he had a normal workup including a normal troponin, ECG, and chest x-ray. H pylori serology was ordered at the time, which is still pending. During this presentation, Mr. Holand was sent home with Gaviscon, which he noted helped relieve his symptoms. He reports that he has not been experiencing any of the substernal central chest pain since Saturday and was not experiencing it this morning either. In addition to the muscle relaxant plus ibuprofen/muscle relaxant plus aspirin combination, Mr. Holand has also been taking naproxen 2 tablets a day approximately 2 to 3 times a week for the last 6 years. He reports taking this for migraines. He reports he occasionally takes Advil; however, not regularly. He does not endorse a significant alcohol use history. He reports drinking approximately 5 drinks per week (beer/cider). He does not report any prior history of peptic ulcer disease. He has no past history of H pylori testing on his medical history. He has not had any prior scopes (upper or lower). Mr. Holand has not noted any fevers, night sweats, or weight loss. His review of systems was otherwise unremarkable. He has been n.p.o. since 9 p.m. last night. At this time, Mr. Holand reports that he is comfortable. He is not experiencing any abdominal discomfort or his usual "chest pain." He does not endorse any nausea at this time. PAST MEDICAL. HISTORY Migraines. Mr. Holand has no other significant past medical history. PAST SURGICAL, HISTORY Vasectomy 5 years ago. MEDICATIONS Mr. Holand does not take any regular prescription medications. He takes the following over-the-counter medications: 1. Muscle relaxant plus aspirin combination/muscle relaxant plus ibuprofen combination for the last 5 weeks, 500 mg tablet 3 to 4 times per day. 2.
