History:
Mark was seen today for a hearing assessment. He reported a history of
working in noise exposure (rail maintenance) and mentioned that he wore
hearing protection diligently. Mark reported that in the past month he
started working in a control room simulator and has been experiencing ear
aches about a week ago due to the constant hum of the computer servers
in the room. The pain was reported to be 6/10 bothersome and
experienced all the time for the past week. Mark also pointed out that the
left ear is more painful than the right. Mark reported some sentivity to loud
noise and a history of migraine. He added that the ear aches has
contributed to more migraines as well. He reported going to the ER after 2
days of constant pain and was referred to us to rule out hearing loss.
Bilateral tinnitus and pressure in the ears was also reported, symptoms
worse in the left ear. Mark denied to any reports of vertigo or recent cold/
congestion.
Otoscopy:
No abnormalities detected, bilaterally.
Hearing Test Results:
Hearing essentially within normal limits bilaterally, mild loss observed at
8000Hz in the left ear. Speech Reception Thresholds (SRT) agree with
pure tone average indicating good reliability.
Tympanometry:
Type A tympanograms bilaterally suggesting normal compliance and
pressure in both ears.
Comments and Recommendations:
Follow up with GP regarding symptoms and test results.
Annual audiological assessment to monitor thresholds and tinnitus.
