SUPPLEMENTARY SUBMISSION Noise Survey Methodology Deficiencies & Evidence of Prior Employer Knowledge WorkSafeBC Claim #42647461 β Mark Holand Submitted: March 19, 2026 EXECUTIVE SUMMARY This submission supplements my March 18, 2026 formal notice to WorkSafeBC and Arcose Consulting Ltd. regarding deficiencies in Report ID 2044-20260306. It presents six specific findings of methodological deficiency in the Arcose noise survey, supported by documentary evidence demonstrating that BCRTC had direct knowledge of my workplace injury, the specific nature of my concerns, and the limitations of their own testing β all prior to commissioning the survey. This submission also documents the systematic progression of my attempts to resolve this matter internally through proper channels, and the employer's failure to act at each stage. | did not report to WorkSafeBC as a first resort. | reported to WorkSafeBC as a last resort, after internal processes failed, my health continued to deteriorate, and | was left with no other option to legally protect myself. The Arcose survey was conducted 53 days after my injury, in an empty room, with an idle system, without access to my clinical injury documentation, and without testing the specific conditions | had reported in writing on January 23, 2026. It cannot serve as a valid clinical assessment of my occupational exposure. DOCUMENTARY RECORD β TIMELINE OF-EMPLOYER KNOWLEDGE The following timeline demonstrates that BCRTC had written, timestamped knowledge of my injury, my specific technical concerns, and my requests for escalation β well before commissioning the Arcose survey. This evidence was available to the employer when they briefed Arcose. It was not provided to Arcose. January 22, 2026 β MyHSE Safety Concern #75625 Filed formally through the employer's own safety reporting system. | identified the server noise as a hazard at Workstation 3, disclosed my personal vulnerability to excessive noise, and explicitly requested the servers be moved. This document was available to Jason Mayo, Austin Puder, and Breanne Douglas.
