WCS ‘acoustic injury' 19Jan2026. Control operator training. No previous hearing issues

Reports noise exposure from servers located behind his desk which he moved to one week prior.
Developed headaches , bilateral earache, irritability and joss of appetite. He filed a safety concern

His employer assessed nose at 56- 63db. File was closed. He also attended ER- no evidence of infection
He remains off work. Ongoing severe anxiety. Headache has improved scores 1/10 improved on nurtec. Left earache
2/10.

Sleep has improved somewhat with seroquel. He is now sleeping in a sleep tent.

Still wearing pads at night- he wets them so he doesn't have to get up. Reports BP has been normal.

imp migraine anxiety disorder unspecified. Plan c/w migraine prevention with coenzyme q10,

would recommend psychiatry/psychology assessment. Remain off work for now

c/w pra nurtec + seroquel 12.5mg nocte. Review in 2 weeks

Headache has improved scores 1/10 improved on nurtec. Ongoing left earache 2-3/10.

Sleep has improved with seroquel up to 8hrs. He is still sleeping in a tent. No longer wearing pads at night.
Not checked BP for a couple of weeks. imp migraine anxiety disorder unspecified.

Pian c/w migraine prevention with coenzyme q10. Would recommend ENT assessment.

c/w prn nurtec + seroquel 12.5mg nocte. Review in 1 week and aim for GRTW at that time
