Subject: URGENT ESCALATION: Unpermitted Industrial Installation & Life Safety Hazard - 6800 14 Ave (OMC1)
Dear Jason Watt, Chief Building Inspector,

| am escalating a formal report of building and fire code violations at the BCRTC Operations & Maintenance
Centre 1 (OMC71), located at 6800 14 Ave, Burnaby.

On March 7, 2026, | submitted detailed photographic evidence to both the Building and Fire Departments
regarding an unpermitted commercial server installation in the Control Training Room. On March 9, 2026, |
received an email from the Building Division stating my concerns were forwarded to the Building Inspections
Supervisor for review and further action.

As of today, March 18, | have not received an update, and this remains an active life-safety hazard.

As detailed in the attached correspondence, this installation involves two refrigerator-sized server racks placed in
an occupied classroom. The critical violations include:

Unpermitted Installation: Direct data access via the Burnaby permit API endpoint confirms no building,
electrical, or mechanical permits exist for this installation.

Change of Occupancy Use: Introducing industrial server racks into a classroom materially changes the
thermal, acoustic, and fire safety profile without the required permit or inspection.

Code 3.1.9 Firestopping Violation: Cables pass through an open ceiling panel with no firestopping.
Because this rack sits directly below the main Skytrain Operations Control Centre, an electrical fire would
have an unobstructed chimney path into a life-safety critical facility.

Electrical Overloads: Circuits serving these racks have tripped breakers due to electrical overload.

This unpermitted equipment is not just a theoretical risk. The acoustic hazard from this installation - specifically
exacerbated by a missing server rack containment panel - has already caused a documented physical injury
(Acoustic Trauma). This injury has resulted in a permanent career disruption, and the regulatory status of this site
is now the subject of an active WorkSafeBC forensic investigation.

WorkSafeBC is currently relying on third-party acoustic reports that treat this unpermitted equipment as a legal,
compliant installation.

| respectfully request immediate confirmation of the following:

1. Has a City of Burnaby Building or Electrical Inspector attended 6800 14 Ave to assess this specific
workstation?

2. Will the City be issuing a compliance order or "Stop Work" notice for this unpermitted Change of Use?
