Hi Austin and Ron,

| will not be returning to work until | receive a diagnosis and treatment, from a specialist. | have
been actively seeking an ENT ( ear nose throat) referral since Jan 30. The medical system has
yet to provide me one, and my attempt to receive emergency care, have resulted in 5
emergency room visits, each time, turned away with out a referral. As a result of my condition
going undiagnosed and untreated, my physical and mental health are declining, do to no rem
sleep.
