My health has continued to deteriorate significantly since the initial claim, and | am formally requesting urgent action. BACKGROUND lam a 25-year heavy industry veteran with a professional background in acoustic environments. On January 22, 2026, | filed a formal Safety Concern (Self-Reporting #75625) identifying an unshielded server rack at Workstation 3, OMC1 Training Room, as a hazardous acoustic environment. The hazard was not remediated. | was knowingly placed at that workstation despite prior awareness of the noise issue. Following three weeks of exposure to mechanical resonance at approximately 328 Hz, reflected and amplified by a glass window directly to my left, | sustained a documented acoustic injury.
