WORKING TO MAKE A DIFFERENCE

Memo
(continued)

Worker last name First name Middle initial WorkSafeBC claim number
HOLAND MARK 42647461

The 4 to 6 week window for treatment of cochlear inflammation and acoustic trauma
is a clinically recognized threshold. That window has now closed. The absence of
timely medical investigation by WorkSafeBC has directly contributed to the risk of
permanent hearing damage.

| am formally requesting that this delay be documented in my claim file, and that an
urgent specialist referral — ENT, neurology, or audiology — be ordered
immediately.

! will be escalating this claim to the Workers' Advisers Office and the WorkSafeBC
Issue Resolution Office if | do not receive a substantive response within 2 business
days.
