Hi Jaylin, | am writing to formally request that WorkSafeBC conduct an independent occupational noise assessment of the worksite at OMC1, Control Training Room, 6800 14 Ave, Burnaby, BC. | am specifically requesting: 1. Full-shift dosimetry — a calibrated dosimeter worn by an occupant of Workstation 3 for a complete 10-hour shift, including periods when the server fans spin up under load. A single point-in-time handheld reading, as previously conducted by the employer, does not capture peak events or cumulative daily exposure. 2. Spectral frequency analysis — measurement of frequency-specific energy levels, particularly in the low-frequency range. My own calibrated recordings identify a significant mechanical resonance at 328 Hz. Standard dB(A) measurements do not capture frequency-specific hazards of this nature. 3. Assessment conducted by a WorkSafeBC occupational hygienist — independent of the employer and the employer's submitted reports. | do not consent to the employer's self-reported noise data being the sole basis for the claim decision. The employer has a direct conflict of interest in this assessment. WorkSafeBC has independent authority and qualified personnel to conduct this testing, and | am formally requesting that authority be exercised. My condition is deteriorating weekly. | have lost approximately 10 Ibs since January, | have documented autonomic symptoms, and | have received no specialist care. The delay in claim adjudication is directly contributing to ongoing harm. Please confirm receipt of this request and advise when the independent assessment can be scheduled. 68B33 Workers’ Compensation Board of B.C. (RO2/09) Page 1 of 5
