Please find attached for inclusion in my claim file my complaint to the City of
Burnaby Building Department dated March 7, 2026, and their response dated March
9, 2026.

The complaint identifies four BC Building Code violations at the OMC1 Control
Training Room — unpermitted installation, firestopping violation under BC Building
Code Section 3.1.9, change of occupancy without permit, and overloaded electrical
circuits. The Building Department forwarded my complaint to their Building
Inspections Supervisor for review and further action.

| filed this complaint after WorkSafeBC's own investigator advised me on February
19, 2026 to report code violations to municipal authorities independently.

These documents have also been uploaded to the WorkSafeBC document server.

Dear Jaylin,

Please find two updated documents attached for inclusion in my claim file.

The first is my updated incident timeline dated March 13, 2026, including all events
through the adverse employment action received yesterday from Austin Puder,
Senior Manager Train Operations.

The second is my updated return to work plan dated March 13, 2026, submitted in
response to the employer's formal removal of the Control Operator training pathway.
This plan requests that WorkSafeBC activate its vocational rehabilitation mandate
and support the BCIT Occupational Health and Safety diploma program as the
appropriate return to work goal.

Both documents have also been uploaded to the WorkSafeBC document server.
