INSPECTION NOTES

This inspection report is issued to verify the employer's compliance with the Workers Compensation Act (WCA) and the
Occupational Health & Safety (OHS) Regulation. | discussed the following with the employer representative, but was not limited to:

Background

This employer is a transit company. The primary focus of this inspection is related to noise in the OCM1 training room. This room
is used for training control operators.

Noise

The employer representative stated that they had conducted noise spot measurements in the OCM1 training room as part of an
investigation regarding noise concerns. The employer representative stated the room contains a server rack which produces some
noise; however, measurements directly at the rack were between 60-70 dBA. | discussed that the employer is only required to
measure noise if workers are or may be exposed to levels exceeding 82 dBA Lex (see reference OHS 7.3(1)).

| am requesting the employer provide me a copy of the following documents:

1.) Investigation regarding noise in the OCM1 training room
2.) Noise measurement data from the OCM1 training room (including device type)
