The medical system has yet to provide me one, and my attempt to receive emergency care, have resulted in 5 emergency room visits, each time, turned away with out a referral. As a result of my condition going undiagnosed and untreated, my physical and mental health are declining, do to no rem sleep. There was a mis communication with my doctor about not requiring a sick note, as he would inform web. | will be talking to my doctor on Monday, and | will have him provide this note. 000262
