June 12. Noted CL approved ee's claim.
OHS confirmed that ee has not provided
any additional medical to BCRTC Occ
health since March 2026. Noted that ee
has filed an appeal of his WSBC claim
decision. LR director advised that ee's
CL WI payments will be withheld until
WSBC appeal decision is reached. LR
director has advised payroll of this
decision already and requested that
OHS update CL. OHS emailed CL CM to
provide update as requested.

Foremed Clinic advising that follow-up
medical questionnaire dated April 2 was
not sent due to ee refusing to give
consent and sign requested employee
consent section. Ee advised the clinic
that the form was not mandatory or
required by BCRTC. Practice Manager
advised that ee was upset that OFA
received by Occ Health on March 26,
2026 was sent by Foremed without his
consent. As such, ee requested to
withhold consent on subsequent medical
questionnaire dated April 2, 2026. OHS
advised that Occ Health had been
awaiting this medical questionnaire and
had emailed it to ee. OHS noted that ee
was provided numerous deadlines by
Labor Relations to provide this form to
Occ Health and elected to be non-
