Email from Canada Life Senior Case
Manager - Erika Nikbin advising that CL
has not received medical information
requested from ee's doctor on May 11,
2026. Case Manager advised that CL
will follow-up and provide deadline to
receive information. As claim decision
hasn't been reached, CL can't suspend
claim. Case Manager to update OHS
regarding claim status if medical
received.

Inquiry from Payroll & Scheduling
regarding ee's tentative return to work
on June 15 and whether ee will be
returning to Control or Guideway
department. OHS advised that ee's
return to work remains largely unknown
and that ee will be returning to
Guideway..

is requesting a copy of his file

42647461. OHC to organize files and
have OHM to review before submitting
