about their condition and treatment plan.
CM has sent request to ee's Physician
for additional medical information. Upon
receiving medical, CM will advised OHS
of decision on claim. CM inquired about
details of any workplace incident that
may have occurred around the time
Mark ceased work, including details of
alleged demotion. OHS provided
requested information and timeline of
events including OFA and Med
Questionnaire issued to ee and
Physician.

Director advised that new deadline of
June 3 to provide medical questionnaire
to Occ health. Should ee not comply
