OHS requested for ee to provide
medical note in PDF format as currently
not accessible and legible for record
keeping purposes. OHS also requested
for ee to send medical information
directly to OHS and to not include LR in
pertinent medical information. OHS also
requested for copy of the completed
follow-up medical questionnaire issued
to ee's Physician on April 2nd.

30. OHS advised of estimated return to
work date of June 15.
