requesting a copy of his file from
January 1, 2022 to March 19, 2026.

Email sent to information analyst at
Translink to advise FOI documents have
been uploaded to OWL.

Email to ee advising that Occ Health is
awaiting completion of medical follow-up
questionnaire sent to Physician's Clinic
on April 2. OHS acknowledged previous
medical note supporting absence till
April 30 and CL claim pending. OHS
provided deadline of April 21 to have
medical questionnaire completed and
attached copy of medical questionnaire.
