Confidentiality Statement

Written retrospectively: Email from email from Kevin Payne -
Director LR to ee with relevant parties cc'd (OHS, LR Advisor,
Union Rep) on May 5.

LR Director advised that new deadline of June 3 to provide
medical questionnaire to Occ health. Should ee not comply
with timeline, company will review his absence to determine if
ee has abandoned his position.

Email from Erika N. - Canada Life Case Manager advising
that she reached out to ee for phone interview however ee
declined to communicate via phone and has requested for
email correspondence only. CM has emailed ee with
questions about their condition and treatment plan. CM has
sent request to ee's Physician for additional medical
information. Upon receiving medical, CM will advised OHS of
decision on claim. CM inquired about details of any
workplace incident that may have occurred around the time
Mark ceased work, including details of alleged demotion.
OHS provided requested information and timeline of events
including OFA and Med Questionnaire issued to ee and
Physician.

Email from Erika N. - Canada Life Case Manager confirming
that ee has submitted WSBC decline letter. Case Manager
to schedule phone interview with ee and then will proceed to
assess their claim.
