BCRTC's own file for March 26, 2026 (14:23), March 27, 2026 (08:53), and March 30, 2026
(10:35).

e Allrecords, notes, and correspondence authored by or sent to Navi Gill (Labour Relations
Advisor) relating to my absence, including the record(s) formally designating me "Absent

Without Leave (AWOL)" on or about April 24, 2026, and any policy or procedure relied upon to
make that designation.

e Allrecords, notes, drafts, and internal approvals relating to the deadline letters referenced in my
termination letter, including the May 5, 2026 letter setting a June 3, 2026 deadline, and any
earlier deadline extensions referenced therein.

e Allrecords, correspondence, and internal approvals authored by or sent to Kevin Payne (Director,
Labour Relations) relating to the decision to terminate my employment, including drafts of the
June 12, 2026 termination letter and any legal or HR sign-off records.

e Invoice #315113 and all associated billing, authorization, and payment records between BCRTC
and ForeMed Medical Clinic / F. Kara Holdings Lower Ltd. relating to me.

e Allrecords in my Occupational Health file, personnel file, and any WorkSafeBC-related
correspondence file held by BCRTC, including the full content of any electronic
case-management system entries (e.g., Cority/Cortico or equivalent) associated with my name or
employee number.

e Acomplete access log / audit trail showing every BCRTC employee or system that viewed,
downloaded, printed, or forwarded my Occupational Fitness Assessment or Medical
Questionnaire records between March 26, 2026 and the present.

e Any privacy impact assessment, breach report, incident log, or internal investigation record
created by BCRTC or TransLink in response to my June 22, 2026 breach notification email to
privacy@translink.ca.

e Allpolicies, procedures, or training materials in effect during this period governing (a) the
collection of medical information from third-party clinics, and (b) the required verification of
employee consent prior to acceptance of such records.

Statutory Timeline

Under section 7 of FIPPA, TransLink/BCRTC is required to respond to this request without delay and in
any event within 30 business days, subject to any permitted extension under section 10 (of which |
request written notice, with reasons, if invoked). Please confirm receipt of this request in writing and
provide the applicable file/tracking number.

If any portion of this request is unclear, or if fees under the Act's fee schedule are anticipated, please
contact me before processing so we can discuss scope. | do not consent to any narrowing of this request
without my written agreement.

Given that this request directly supports an active OIPC complaint, | would ask that a copy of your
response and the responsive records also be retained for production to the OIPC investigator upon
request.
