To the FOI Coordinator,

Pursuant to section 5 of the Freedom of Information and Protection of Privacy Act (FIPPA), and in
support of a related complaint currently before the Office of the Information and Privacy Commissioner
for British Columbia (OIPC), | am formally requesting access to all records in the custody or control of
TransLink / BCRTC that relate to me, my employment, and the disclosure and handling of my medical
information described below.

This request concerns the unauthorized disclosure of my Occupational Fitness Assessment (OFA) by
ForeMed Medical Clinic to BCRTC Occupational Health on March 26, 2026, and the subsequent use of
that information in decisions culminating in the termination of my employment on June 12, 2026. |
require the records below both to substantiate the OIPC complaint and to fully understand how my
personal health information was collected, used, stored, and disclosed by BCRTC.

Records Requested

lam requesting complete, unredacted (as against me, the subject individual) copies of the following, for
the period January 1, 2026 to present, unless otherwise noted:

e Allemails, faxes, memos, and internal correspondence sent or received by Adebayo Adenekan
(Occupational Health Specialist) referencing me, including but not limited to the fax cover sheet
and Medical Questionnaire dated April 2, 2026, and the original OFA referral sent to ForeMed
Medical Clinic / Dr. Ademiluyi on or about March 24, 2026.

e The complete Occupational Fitness Assessment (OFA) record received by BCRTC from ForeMed
Medical Clinic on March 26, 2026, including fax transmission logs, header/footer metadata, and
any confirmation-of-receipt records.
