the fax cover sheet and Medical Questionnaire dated April 2, 2026, and the original OFA
referral sent to ForeMed Medical Clinic / Dr. Ademiluyi on or about March 24, 2026.

The complete Occupational Fitness Assessment (OFA) record received by BCRTC from

ForeMed Medical Clinic on March 26, 2026, including fax transmission logs,

header/footer metadata, and any confirmation-of-receipt records.

All internal call logs, notes, or records of outbound/inbound contact attempts to me by

Occupational Health or Labour Relations staff, specifically including the entries

referenced in BCRTC's own file for March 26, 2026 (14:23), March 27, 2026 (08:53), and

March 30, 2026 (10:35).

All records, notes, and correspondence authored by or sent to Navi Gill (Labour

Relations Advisor) relating to my absence, including the record(s) formally designating

me "Absent Without Leave (AWOL)" on or about April 24, 2026, and any policy or

procedure relied upon to make that designation.
