As part of our response to this incident, we have reviewed and strengthened our intemal Privacy procedures. We
have revised our consent policy for third-party disclosures, implemented a mandatory checklist for staff to
complete before reteasing patient information, and provided additional Privacy training fo our entire team to help
prevent a similar incident from occurring in the future.

the College of Physicians and Surgeons of British Columbia, we will cooperate fully with any inquiry.
