3. Impact on BCRTC's Credibility and Institutional Pattern

The clinic's written confession changes the legal and administrative posture of both parties in
several critical ways:

The "Smoking Gun" on Bad Faith Procurement: BCRTC did not merely
receive records via an unsolicited clerical mix-up from a doctor's office.
BCRTC's Occupational Health department actively initiated the exchange using
a pre-printed cover narrative asserting that a signed authorization form was

attached. BCRTC knew, or ought to have known, that no such signature existed
on Part 3 of that form.

1. Corroboration of the Threat Continuum: This admission directly indicts the
subsequent actions of Labour Relations Advisor Navi Gill (April 24 Notice to Comply)
and Director Kevin Payne (May 1 Termination-Jeopardy Letter). Both corporate
actors used an admittedly unauthorized, illegally procured medical file as their explicit
evidentiary foundation to designate a 2.9-year employee as AWOL and threaten his
career.

2. Establishment of Administrative Deception: Bypassing patient consent by
submitting a blank form while textually claiming authorization is enclosed mirrors the
exact administrative behavior documented in the “Address Shell Game" (Section 3)
and the "Staged Test Environment" (Section 6). In all instances, BCRTC
Management prioritized generating an advantageous paper trail over compliance with
underlying regulatory and legal obligations.

4. Supplemental! Timeline Entry
The consolidated timeline in Section. 11 is hereby updated to include the final closure of the
PIPA verification loop:

e July 17, 2026 (7:27 PM) | Actor: Parween Yar (Clinic Manager, ForeMed) | Event:
Issues final PIPA investigation response via secure email; formally apologizes for the
“unauthorized disclosure," confirming the file was sent in error and validating that
BCRTC failed to provide the signed authorization claimed in their paperwork. |
Source: Exhibit X (Cortico Secure Thread).

5. Supplemental Exhibit Index Entries

The Master Exhibit Index in Section 13 is appended to include the following core evidence:

e Exhibit X: Formal PIPA Investigation Outcome Email from Parween Yar (ForeMed
Clinic Manager) to Mark Holand, dated July 17, 2026, 7:27 PM, admitting
unauthorized fax transmission to BCRTC and detailing systemic procedural failures.
