ADDENDUM TO THE CREDIBILITY
RECORD
Conclusive Evidentiary Confirmation of Section 10
Privacy Breach (The ForeMed Admission)
Prepared by: Mark Thomas Holand
Prepared for: Master reference record for adaptation to WorkSafeBC Review Division,
OIPC, and related proceedings
Date: July 17, 2026
1. Overview of Addendum
This addendum serves as a supplement to Section 10 ("The Privacy Breach and
Weaponization of Leaked Medical Information") of the primary credibility dossier. On July 17,
2026, at 7:27 PM—less than 36 hours after the primary record was consolidated—ForeMed
Medical Clinic issued a formal, written admission confirming that the medical data
transmission on March 26, 2026, was an unauthorized disclosure executed entirely
without patient consent.
This admission transforms Section 10 from a Strongly corroborated inference into an
uncontested matter of objective fact. It.establishes a definitive paper trail proving that British
Columbia Rapid Transit Company (BCRTC) actively exploited administrative blind Spots to
extract confidential medical files under false pretenses.
2. The Core Admissions by ForeMed Medical Clinic
In a formal review signed by Clinic Manager Parween Yar, ForeMed documented the
systemic failures that led to the breach:
e Explicit Confirmation of the Leak: The clinic explicitly verified that the worker’s
confidential Occupational Fitness Assessment (OFA) Form was “faxed to BCRTC in
error" and describes the incident as an “unauthorized disclosure".
e Reliance on the BCRTC Corporate Bluff: ForeMed confirmed that BCRTC’s
documentation contained an explicit misrepresentation, noting that the third page of
the form provided by BCRTC stated, “Please find included the completed
Authorization form allowing disclosure of this information to BCRTC Occupational
Health." ForeMed's internal investigation concluded that while the form asserted an
authorization was included, “that did not occur in this instance."
e Bypass of Mandatory Verification: The clinic admitted that its staff member
processed the file based on an "assumption" of consent. The staff member
substituted completely unrelated, historical consent forms on file for Canada Life
Insurance and WorkSafeBC as justification to bypass checking the blank patient
authorization field on the current BCRTC document.
e Admission of Policy Failures: As a direct result of this specific breach, ForeMed
disclosed that they have been forced to implement a mandatory checklist for staff
