June 22, 2026 ~ placing both organizations on an identical thirty-business-day statutory response clock
under PIPA and FIPPA.
Evidence: Exhibit V— BCRTC Phone Log 1.jpg (May 26, 2026 tracking call). Exhibit W — OIPC/ForeMed/Transtink
privacy complaint correspondence, June 22, 2026.
As with Sectians 2-9, above, no single item here would be dispositive alone. Together, they show BCRTC
representing that a signed consent existed when its own file shows-the consent field blank; using a
financial incentive to move a physician’s office to fax quickly; and then using the resulting file — built ona
form BCRTC’s own hand received unsigned — as the evidentiary foundation for an AWOL designation and
a threat to end’a 2.9-year career. It is the same institutional pattern documented throughout this dossier:
the paper trail, not the underlying safety or medical reality, is what BCRTC appears to have managed at
each point of outside scrutiny.
