As part of your employer's disability management program, Canada Life has reviewed the medical
information you've submitted to determine whether your condition would prevent you from working. Based
on the information we received, your absence is medically supported from February 2, 2026 to August 3,
2026. We have notified your employer of our recommendations.

Weekly Indemnity definition

Disability is assessed on the basis of the duties you regularly performed for your employer before disability
started. An employee will be considered totally disabled on any day the employee is continuously unable
due to illness to do the essential duties of the employee’s own occupation.

The maximum date for your Weekly Indemnity claim is August 3, 2026. Long-Term Disability (LTD) starts
effective August 4, 2026. We are currently in the process of reviewing your eligibility for LTD benefits. We
will advise you of our decision once our assessment is complete.

If you have any questions, you can reach me at (604) 648-3094.

Any personal information provided will be used as set out above and in accordance with our personal
