| am the Occupational Health Specialist managing Mark Holand’s absence and return to
work.

| have you listed as the case manager as per Jenna O’Neill for Mark Holand (E21987). |
understand the claim remains partial and is still being adjudicated. We received an
Occupational Fitness Assessment from Mark’s Physician who advised that he wouldn’t
be able to commence a GRTW for another 2-3 months. The attached follow-up medical
questionnaire was issued to Mark’s Physician on April 2 and we've yet to receive a
response.

Mark Holand is also not responding to emails or phone calls over the last 1 month.
Do you have any further medical supporting his absence beyond April 30?
