4. COOLING PROBLEMS & EQUIPMENT STATE

All records relating to cooling problems, overheating, fan noise, circuit overloads, or the
removal or absence of side panels from the server rack equipment, including any
communications authorizing or documenting the removal of panels or the placement of
supplementary cooling equipment such as portable fans.

5. PERMITS & REGULATORY COMPLIANCE

All records relating to building permits, electrical permits, Technical Safety BC
approvals, fire suppression requirements, or any other regulatory compliance
documentation sought or obtained for the OMC1 server installation. Including any
records indicating that permits were not obtained or that the installation did not proceed
through normal permitting channels.

6. COMMUNICATIONS REGARDING MARK HOLAND

All internal communications referencing employee Mark Holand in relation to the OMC1
server installation, Workstation 3, MyHSE Safety Concern #75625, the acoustic injury
claim, the Arcose noise survey (Report ID 2044-20260306), or the adverse employment
action letter dated March 13, 2026.

7. COMMUNICATIONS WITH ARCOSE CONSULTING

All records of communications between BCRTC and Arcose Consulting Ltd. regarding
the noise survey conducted at OMC1 on March 6, 2026, including the original brief or
instructions provided to Arcose, all correspondence before and after the survey, and
any records indicating what documentation was or was not provided to Arcose.

8. NOVEMBER 2025 TRAINING CLASS

All records indicating whether noise or equipment concerns at Workstation 3 were
identified or communicated during or after the Control Operator training class that
commenced November 17, 2025.

| am requesting records in electronic format where available. | am a worker on unpaid
medical leave with no current income and request that any fees be waived on the basis
of financial hardship and public interest — this request relates to an active workplace
injury claim and ongoing regulatory investigations.
