To the Information Access Department,

| am submitting a formal request for access to records under the Freedom of
Information and Protection of Privacy Act (FIPPA), relating to BC Rapid Transit
Company Ltd. (BCRTC) and the installation and operation of server rack equipment in
the OMC1 Control Training Room, 6800 14th Ave, Burnaby, BC.

| am the subject of an active WorkSafeBC claim (#42647461) arising from acoustic
injury sustained at Workstation 3 in the above location beginning January 12, 2026. The
records requested are directly relevant to that claim and to related regulatory complaints
currently under review by the City of Burnaby Building Inspections Division.

| am requesting all records in the custody or control of BCRTC and/or its operating
companies relating to the following categories, for the period January 1, 2022 to
March 19, 2026:

1. PLANNING & INSTALLATION

All records relating to the planning, procurement, design, and installation of server rack
equipment in the OMC1 Control Training Room, including scope of work documents,
contractor communications, project timelines, and any decisions regarding the
placement of server racks at or near Workstation 3.

2. NOISE, HEAT & SAFETY ASSESSMENTS

All noise assessments, heat assessments, risk assessments, or safety reviews
conducted prior to or following the installation of server equipment in the OMC1 Control
Training Room, including any records predating the installation that identified noise or
heat as a concern.

3. INTERNAL COMPLAINTS & SAFETY CONCERNS

All internal communications regarding noise levels, employee complaints, safety
concerns, or work refusals relating to Workstation 3 or server equipment in the OMC1
