Good Morning,

We've received the attachments from this employee and asked him to complete the form A. So
far we have not received form A or any response back so the attached receipts are pending to
be processed. To add, EE sent Employee’s Statement, Attending Physician’s Statement and
Consent Form to Canada Life and they also confirmed they don’t have a form A.

We have received another letter from Canada Life and they claim that based on the medical
submitted for the absence on or around January 29, 2026, it has been determined the absence
was “work-related”. Could you please confirm if a claim has been submitted to WorkSafeBC for
consideration?
