| am not medically cleared to return to my role as a Guideway Serviceperson on March
30. Attached is the latest medical documentation from my physician confirming my
continued status of Total Disability.

Due to the severity of the condition—which recently required emergency transport via
ambulance—my physician is unable to complete a fitness assessment or address the
Job Demands Analysis at this time. We are currently awaiting necessary specialist
intervention to determine a treatment plan and future prognosis.

| have CC'd the Guideway department to ensure their records are updated and to
confirm | will not be reporting for duty on the 30th. | will provide further updates once a
specialist has been consulted.
