Mark Holand and his Physician’s office was issued a medical questionnaire/occupational fitness assessment on March 24 to be completed by his Physician. He has however provided a generic medical note that doesn’t outline his current restrictions or limitations or explain the reason for his total disability. He remains on an unpaid medical leave and has advised his Guideway department. Just wanted to keep you in the loop. Thank you, ADEBAYO ADENEKAN, DPhty., B.Kin (Pronounced “Aday-Bio?) Attached is the latest medical documentation from my physician confirming my continued status of Total Disability. Due to the severity of the condition—which recently required emergency transport via ambulance—my physician is unable to complete a fitness assessment or address the Job Demands Analysis at this time. We are currently awaiting necessary specialist intervention to determine a treatment plan and future prognosis. 000091
