Hi Mark,

Thank you for your email. While | acknowledge the medical note provided and your
approximate return to work on April 30 indicated by your Physician, Occupational
Health requires objective information from your Physician to better understand your
functional capacity for your Guideway Serviceperson role. There are specific questions
in the Occupational Fitness Assessment that will assist us in return to work planning.

| acknowledge that you’re awaiting necessary specialist intervention to determine a
treatment plan, however | would appreciate the completion of the Occupational Fitness
Assessment form by your Physician prior to best understand your current function in
relation to your Guideway Serviceperson role.
