We received an OFA on March 26 advising that his prognosis for return to work was
“unknown but likely 2-3 month”. A follow-up medical questionnaire was issued to his
Physician’s office on April 2 and we’ve yet to receive a response.

The same medical questionnaire was emailed to Mark on April qqth inquiring about his
next scheduled appointment with his Physician to which | received no response. |
provided him with a deadline of today, April 21 to get back to me.

| am unable to leave a voicemail on his phone and | have received any correspondence

from him since his last medical note he provided to myself and cc’d Guideway Managers

Nick & Cam on March 25 noting his medical absence until April 30.

Mark advised in early March that he prefers to receive correspondence via email, but has

been AWOL as of the last month.

Can you please follow-up with him and take your necessary course of action?

Thank you,
