Note T/C with EE advising that he's been scheduled to resume work Friday March 6 (night shift). Ee noted that he
remains significantly unwell and is actively seeking treatment. OHS noted that previous medical note supports
absence till March 5 only, Ee advised that his doctor said he doesn’t need to provide a medical note and WSBC
has been advised that ee remains unfit to rtw. OHS advised that Occ Health requires medical supporting absence
in order to inform department adequately. Ee was agitated and frustrated but demonstrated understanding and
plans to obtain medical note on Monday March 9, Ee noted that he's attending Worker's advisory office in
Richmond to ladge complaint regarding the manner in which his case is being managed. Ee frustrated that he's
been refused specialist ENT review.

due to being significantly unwell and is actively seeking treatment. OHS ‘advised for scheduling to remove ee until
March 13 out of caution until medical note obtained.

Note Email from ee to Control Manager - Austen P.-& Rail Ops Manger ~Ron W. advising that he's actively seeking an
ENT and has yet'to receive formal diagnosis and treatment. Ee also highlighted: miscommunication with his
doctor who wrongly advised that he doesn't tequire a sick note. Ee to update medical note from doctor today and
will provide to Occ health upon receipt.

Note During Pulse Check, Navi G. - LR advisor communicated that ee would be returning to Guideway due to
unsuccessful completion of Control Operator training.

Note T/C with ee who reported some improvement in condition particularly following administration of sleep medication,

Ee reported ear remains sensitive to sound. Ee discussed tentative return'to work on March 29th to Guideway
with conditions. Ee noted that due to ongoing issues affecting circadian rhythm he would not be appropriate for
night shifts and will be seeking to only complete office-based work during the day. OHS advised that ee would
need to provide medical to support such arrangement.

Ee also noted filing grievance following recent termination from Control Operator position due to unsuccessful
completion of trainina.
