This unauthorized disclosure was unintentional and does not reflect the standard of care we strive to provide to our patients. As part of our response to this incident, we have reviewed and strengthened our internal privacy procedures. We have revised our consent policy for third-party disclosures, implemented a mandatory checklist for staff to complete before releasing patient information, and provided additional privacy training to our entire team to help prevent a similar incident from occurring in the future.
