1. Emotional distress related to work situation:

- Continue counseling.

- Await appointment with psychiatrist for further assessment and management.

2. Persistent fatigue:

- Await specialist evaluation for diagnosis and management.

3. Otalgia:

- Await ENT and otolaryngologist appointments for further evaluation and diagnosis.
- Attempt to get on waiting list for earlier otolaryngologist appointment.

Additional:

- Patient will not return to work until a specialist evaluation is completed and a diagnosis is established.

WCB ‘acoustic injury’ 19Jan2026. Control operator training. No previous hearing issues

Reports noise exposure from servers located behind his desk which he moved to one week prior.

Developed headaches , bilateral earache, irritability and loss of appetite. He filed a safety concern

His employer assessed nose at 56- 63db. File was closed. He also attended ER- no evidence of infection

He remains off work. setraline risperidone and trazodone. He is sleeping better. He saw an audiologist and was advised
he had "hyperacusis'

Was recommended sound therapy. WCB claim has been denied. He is appealing decision. Headache resolved. Ongoing
left earache 4-5/10 (stable).

He is still sleeping in a tent with weighted blanket. imp migraine, MDD with anxious distress. Plan c/w sertraline
risperidone and trazodone.

Not fit for GRTW at this time. Have referred to ENT and neurologist via MSP. Refilled meds. R/v in 2 weeks
