Ss:

Emotional distress:

- Experiencing emotional episodes.

- Received a demotion letter from work, causing shaking throughout the body.

- Believes demotion is retaliation for reporting fire code and building permit violations at work.

- Reported violations to the city of Burnaby on February 19th after WorkSafe declined to address fire code issues.
- Feels backed into a corner and unsupported.

- Calmed down after initial upset.

- Not in a mental state to return to work.

- Experiencing panic attacks.

- Currently receiving counseling.

- Awaiting an appointment with a psychiatrist through Royal Columbia mental health support.

- Saw a psychiatrist in emergency who prescribed sleeping medication, but not on a regular basis.
- Has not had a formal mental health assessment.

Fatigue:

- Significant fatigue, limiting activities such as skiing and walking the dog.
- Unable to perform physically demanding work.

- Considers fatigue a primary reason for not returning to work.

Sleep disturbance:
- Sleep has improved, now experiencing REM sleep and remembering dreams.

Ear pain:

- Persistent ear pain.

- Has an ENT referral from St. Paul's, awaiting a date.

- Has an otolaryngologist appointment scheduled for May 27th, attempting to get on a waiting list.
- Unable to return to work without a diagnosis from a specialist.

General:
~- Not returning to work until seeing a specialist and receiving a diagnosis.
- Training program at work has been shut down.
