Initial assessment period
During the initial assessment period, which consists of the 184 day waiting period plus the next 24 months
of disability, a person is considered disabled if:

1. Disease or injury prevents them from performing the essential duties of their regular occupation.

You will receive benefits during the initial assessment period if you satisfy the definition of disability and all
other requirements of your plan. Your initial assessment period will end on August 3, 2028.

Subsequent assessment

After the initial assessment period, you'll continue to be entitled to benefit payments if you’re unable to be
gainfully employed in any job. We'll provide you with more information about the subsequent assessment
later in your initial assessment period.

Rehabilitation and Medical Coordination services

These services may be available to help with your recovery and assist you in returning to work. Your plan
may pay for reasonable expenses associated with participating in a rehabilitation or medical coordination
plan developed and approved by us. We may call to discuss these services with you and explain how they
work.

Your responsibilities
To continue receiving your LTD disability benefits and help with your recovery, you must:

« Participate in your treatment plan and other medical activities appropriate for your condition(s).

e Beunder the care of a legally licensed Doctor of Medicine, including a certified specialist if we
have requested one.

e Work with your health care provider to address barriers to treatment, difficulties accessing

treatment and side effects of medications.

Tell us of any changes in your medical status or treatment plan, including changes to medications.

Take part in rehabilitation or medical coordination plans we recommend.

Take part in a gradual or other return to work plan developed for you.

Provide medical or other information when requested by Canada Life.

Please return the following form(s) we’ve included with this letter in the next 15 days:

e Reportable Income and Income Declaration Form; and
e Worker’s Compensation Reimbursement Agreement Form.

As outlined previously, your claim is currently approved to August 31, 2026; benefits beyond this date are
pending receipt of additional medical information. We will be sending a request to your doctor for updated
medical information in early August 2026. We will advise you once our request has been sent out. Upon
receipt of the medical information, we will be in touch regarding the status of your claim beyond August 31,
2026.
