Approval for Long Term Disability benefits

We’ve approved your claim for Long Term Disability (LTD) benefits for your absence commencing
February 2, 2026 to August 31, 2026. To help you understand how your disability benefits work, we’ve

Workers’ Compensation Reimbursement Agreement
We understand that you filed a claim with WorkSafe BC (WSBC) and your application was denied. We
acknowledge receipt of your WSBC declination letter.

Please refer to the attached Workers’ Compensation Reimbursement Agreement. Please sign and date
this form and return it to our office by email, fax or mail at the addresses / numbers listed under our above
header. Upon receipt of your signed Reimbursement Agreement, we will proceed with issuing your benefit
payments.

The amount we pay

Your LTD benefits will begin on August 4, 2026 following your 183 day waiting period that started on
February 2, 2026 and ended on August 3, 2026. Your first LTD payment covering the period of August 4,
2026 to August 15, 2026 will be issued under separate cover upon receipt of your signed Reimbursement
Agreement and is due to be received by August 15, 2026. Future semi-monthly LTD payments will be
issued by the 15" and last day of each month.

If you would like to have the direct deposit of your benefit payments set up, please submit a copy of a
cheque marked “void” or a printout of your bank account information to our office at the
addresses/numbers listed under our above header.
