assist with the search, | have attached my original FOI
submission from March 19, as well as the official header from
my WorkSafeBC Clinical Opinion file.

1. The Number: As shown in the attached WorkSafeBC
document, 42647461 is my active WorkSafeBC claim number. If
you review the attached March 19 correspondence for FOI
Request 2026/143, you will see that | explicitly provided this
exact claim number in the second paragraph of my original
submission.
