Further Request for Medical Information and Absence Without Leave
We write further to our recent correspondence and for the purpose of reiterating our request for
medical information. By way of background:
a.
You have been off work continuously since approximately January 29, 2026.
On March 9, 2026, you provided a medical note which simply stated that you were
experiencing a “medical problem,” were “unable to work due to medical reasons” and
provided an estimated return to work date of April 30, 2026.
On March 24, 2026, and for the purpose of obtaining further and better information about
your medical circumstances, we wrote to your physician, Dr. Ademiluyi, and asked that he
complete a medical questionnaire;
On March 26, 2026, Dr. Ademiluyi, provided us with a completed medical questionnaire in
which he advised, among other things, that you were off work due to an occupational injury
and unable to return to work for at least two to three months;
Since receiving this questionnaire, we have taken repeated steps to obtain further and
better medical information from your physician about the answers given by him and your
ability to return to work with or without a workplace accommodation. Specifically, our
records indicate that requests for such information have been made to you and your
physician via phone and/or email on March 30, April 2, April 14, April 15 and April 21.
Such attempts were largely, if not completely, ignored, until we wrote to you on April 24,
2026, and advised as a result of your failure to provide sufficient information, you were
currently considered absent without leave (“AWOL”). In this letter, you were also directed
to provide us with a completed version of the questionnaire previously sent to you by no
later than 4:00pm on Thursday April 30, 2026.
