Dear Mark,

Re: Notice to Comply and Communicate with the Company

According to the Company’s records, you have been off work since January 29, 2026. BCRTC’s
Occupational Health Department (“OH”) has been in contact with you, and we understand that the
Company received the Occupational Fitness Assessment on March 26, 2026. A follow-up medical
questionnaire was sent to your general practitioner’s office on April 2, 2026. In addition, the same
questionnaire was emailed to you on April 14, 2026 with a request to confirm the date of your next
appointment with your general practitioner. You were to provide this information by end of day, April
21, 2026.

Despite attempts to contact you by phone on March 30, April 16, and April 21, 2026, OH has not
received any correspondence from you since your last medical note which was submitted on March
25, 2026. Given you have not met the established deadline, your current absence is not supported
with sufficient medical information. As a result, you are considered absent without leave.

This letter serves to inform you that you are required to comply with the Company’s standard
process for medical absences, which applies to all employees, including maintaining reasonable
communication with the Company regarding your absences and the submission of medical
information to substantiate one's absence from work.

You are instructed to initiate communication and to provide the Company with the completed
medical questionnaire from your general practitioner to Bayo Adenekan, OH Specialist (604-902-
0253) by no later than 4:00pm on Thursday, April 30, 2026.

Please be advised that failure to comply may result in discipline, and or a review of your
employment status.
